SC Crew Registration Forms

2012-2013  Registration Forms Bundle - which includes the 9 forms listed below (left click on link to open immediately or right click to save....it can take a few minutes to download). If you need to print a particular form, just click on the form name below to download it.

1.  SC Crew Registration Form

2.  FCPS Emergency Care Form

3.  SC Crew Emergency Care Addendum (complete before Winter Conditioning, if participating)

4.  FCPS Field Trip Driver's License and Vehicle Insurance

5.  FCPS Parental Authorization and Acknowledgement of Risk (aka Field Trip Permission Form)

6.  SC Crew Rowing Release of Liability

7.  VHSL Physical Form (turn into SCSS Activities Office) (complete before Winter Conditioning, if participating)

8.  SC Crew Photo Release Form

9.  SC Crew Swim Certificate (not required for returning rowers)

10.Concussion Test (online link) Needs to be completed by both Parent & Rower

PLUS, in addition to the 9 (yes 9!) forms above:

a US Rowing waiver form must be completed online and forwarded to SC Crew.  Enter Roster code JST6M.  It must be on file prior to participation in any SC Crew activities (including Winter Conditioning and Learn-to-Row) and must be completed again in January of the season year, and...

NOTE:  VHSL Physical Form:  For the 2012-2013 season, the form must be based on a physical conducted after May 1, 2012.   Submit completed/SIGNED form to SCSS  Student Activities office (near gym).
